Target Audience
This activity is intended for Physicians, Physician Assistants, Nurse Practitioners, Registered Nurses, Healthcare Executives, Social Workers, Case Managers, and Resident Care Facility/Nursing Home Administrators engaged in Care Transitions and Readmission Prevention of patients.

 

Educational Objectives
After completing this activity, the participant should be better able to:

Establish a knowledge base of a comprehensive readmission prevention plan for organizations to prevent avoidable hospitalizations.
Describe the core skills necessary to construct a comprehensive bundled payment program that focusses on quality care delivery from hospital to nursing facility to home, in a cost efficient manner.
Effectively communicate transitioning patients in levels of care from hospital to skilled nursing facility to home environment.
Discuss the advantages and utility of community-based programs available at Federal and State levels to assist in the efficient transition to value-based models.

 

Physician Continuing Medical Education
This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Postgraduate Institute for Medicine and _ National Readmission Prevention Collaborative. The Postgraduate Institute for Medicine is accredited by the ACCME to provide continuing medical education for physicians. The Postgraduate Institute for Medicine designates this live activity for a maximum of 4.0 AMA PRA Category 1 Credit(s)™. American College of Healthcare Executives
The Postgraduate Institute for Medicine is authorized to award 4.0 hours of pre-approved ACHE Qualified Education credit (non-ACHE) for this program toward advancement, or recertification in the American College of Healthcare Executives. Participants in this program wishing to have the continuing education hours applied toward ACHE Qualified Education credit should indicate their attendance when submitting an application to the American College of Healthcare Executives for advancement or recertification.
